New patients can be enrolled from the Lara Health portal on a computer or a tablet. Follow these steps to enroll your patients.
- Open the ENROLL tab on the left or use the ENROLL button on the top of your browser window.
- Enter the patient first and last name, email address and mobile number. Assign each patient to a care team such as their lead doctor or a designated staff member.
It is important you provide ACCURATE information during the enrollment so the patient can successfully use the app and receive important message during the program.
- Mark which device was provided to the patient, if any. It is important you enter the correct SIM device number when a cellular device is provided to the patient. Failing to do so could result in data loss.
Ensure the Bluetooth device is properly charged before its first use.
- Mark if monitoring device setup or education on its use was provided. This will create a reimbursement claim entry automatically in the patient’s billing log.
- Click the ENROLL button.
The patient will receive a text message with instructions to download Lara Health for Patients app. They will be able to log into their app using the email address entered during the enrollment and the password sent via the SMS.
