Once the patient’s enrollment has been completed, a patient profile is created in the portal. Currently 4 sections are enabled: patient key information, patient details, patient vitals and billing.
If you made a mistake during the enrollment, need to reassign a device, or the patient’s details have changed, you need to edit the patient’s information accordingly.
Patient key information
Patient’s key details are displayed in the upper portion of the patient’s record. To update patients’ key details, click the pencil icon and save changes by clicking the save icon afterwards. A message that the patient’s profile has been updated successfully will appear.
Please do not change the patient’s email as it will disable the patient’s login credentials for their Lara Health for patient app. Please contact our support team to assist you with changing a patient’s email.
When to change patient’s details
The portal allows users to add patient information such as their insurance provider, home address or EHR ID, and it also helps track their monitoring devices management and enrollment in care programs. You can, and should, make changes to the patient profile as necessary, to keep it up to date. Here are a few examples when you will need to do so:
- If a patient is being enrolled to another care program or removed from an existing one. Incorrect or out of date details might affect their reimbursement eligibility. For more information GO HERE
- When a patient is being assigned a SIM device from another patient. For instructions GO HERE
- When an existing patient needs another (additional) monitoring device. For instructions GO HERE
- When a patient changed their phone number as you might not be able to contact them via the portal.
